What to Bring to Your Visit

Please bring the following items with you:

  • Completed Patient Forms
    If you submitted these online, you do not need to bring paper copies.

  • Driver’s License or Photo ID

  • Vision Insurance Card

  • Medical Insurance Card
    Needed if your visit involves treatment for a medical condition affecting your eyes.

  • Insurance Subscriber Information
    If your coverage is under a spouse or another family member, please provide their name, date of birth, and the last four digits of their Social Security number.

Insurance and Payment Policies

  • We make every effort to pre-authorize your benefits before your appointment. Please complete your forms in full and submit them electronically or bring them with you so we can verify your coverage accurately.

  • Insurance cannot be backdated. Some plans renew eligibility by calendar year, while others renew based on your previous date of service. If you are unsure how your plan works, please call the member services number on your insurance card.

  • Co-pays are due at the time of service. We accept:
    American Express, Discover, Mastercard, Visa
    You may also use your FSA or HSA card to pay for exam co-pays, glasses, or contact lenses.

Frequently Asked Questions

Why do I need to provide my medical insurance card?

  • We keep your medical insurance on file in case your visit involves a medical condition. Diagnostic scans or imaging performed by Dr. Fiorenza are typically billed to medical insurance rather than vision insurance. Eye infections or other urgent medical issues are also billed through your medical plan.
     

What does vision insurance cover?

  • Vision insurance typically covers one annual exam, usually with a co-pay of around $10 for the exam and refraction.
    If you have one of the major vision plans we accept, we will pre-authorize your benefit and provide a full breakdown of covered services and allowances for glasses or contacts. Vision plans usually offer discounts on eyewear, and frame eligibility may renew annually or every two years depending on the plan.
    Most plans allow you to use your benefit for either glasses or contacts, not both. In many cases, patients save the most by applying benefits toward glasses and using any available manufacturer rebates when purchasing a one-year supply of contacts.
     

I have new insurance. How do I know what is covered?

  • The best first step is to call the customer service number on your insurance card. Coverage varies widely, and while we are familiar with many plans, we may not know the specifics of every individual policy. If you have questions about your bill or how your discounts and co-pays were applied, please ask—we are always happy to help clarify.
     

My insurance provider lists Montgomery Vision Care as in-network, but I don’t see it on your list. What should I do?

  • Please give us a call. Insurance companies do not always keep their provider directories updated, and we may be listed incorrectly. We can quickly confirm whether or not we participate with your plan.
     

Is Optomap Retinal Imaging covered by insurance?

  • No. The fee is $35, and payment is due at the time of service.
